This policy outlines the principles, responsibilities, and procedures our organization follows to manage personal information in compliance with legal and ethical standards.

To protect and respect participants’ privacy and dignity while accessing services, ensuring compliance with legal obligations and ethical practices.

2. Scope

This policy applies to all staff, management, and volunteers at Salams Therapy Hub.

3. Policy

Salams Therapy Hub upholds the privacy and dignity of participants, staff, and other stakeholders by:

Safeguarding personal information throughout its collection, storage, and handling processes.Ensuring participants are informed about the purpose and usage of their personal information and obtaining their consent.Following the NDIS Practice Standardsand Australian Privacy Principles to protect the confidentiality of sensitive information.Making all policies accessible in formats suitable to participants’ communication needs, including easy-read documents and interpreter services if required.

Providing information on their privacy and confidentiality rights.Ensuring interviews and discussions on sensitive matters are conducted in a private and respectful manner.Only collecting information that is necessary for service provision and maintaining its accuracy and security.

4. Procedures

4.1 Handling Personal Information

Ensure privacy during discussions about personal or sensitive matters.Collect only necessary information using fair and lawful methods, with the participant’s consent.Keep all records accurate, up-to-date, and securely stored.Destroy or permanently de-identify personal information no longer required, in accordance with legal retention policies. Notify participants when personal information is being recorded in any format (audio, visual) and obtain written consent.

4.2 Managing Participant Records

Confidential records are accessible only to staff involved in direct service delivery. Third-party access to participant information requires prior written consent from the participant or their advocate/guardian. Hard copy files are stored securely in locked cabinets within the director’s office.

4.3 Staff Responsibilities

All staff are responsible for safeguarding personal information they access. The director oversees compliance with privacy standards, ensures staff are trained on privacy protocols, and addresses privacy-related complaints.

4.4 Privacy Information for Participants

Participants will be informed during their first interview about the type of information collected, its purpose, and their rights to privacy. Participants can opt out of NDIS information-sharing audits if they choose.

4.5 Privacy in Interviews and Discussions

Personal or sensitive discussions will only involve information necessary for support provision. Records are protected from unauthorized access, misuse, or disclosure through reasonable security measures. No personal information will be shared with third parties without explicit consent unless required by law.
